STEP 1: Upload your test result & photo ID

Create your AZOVA account and upload a photo of your COVID-19 test result and government issued photo ID.

Note: If you are uploading a result on behalf of a dependent, first create your own account and then add them as a dependent.

STEP 2: We will confirm each COVID-19 test result that you upload
Your digital COVID-19 test record will be confirmed and we will automatically create your COVID Credentials™ Simple Health Pass for you. You will be notified via SMS and email with a link to view your health pass and Credential ID.
STEP 3: We will automatically share your test result with your consent.
We will automatically share your test result with your consent. You can use your health pass for work, travel, school and entertainment. Anytime you take another COVID-19 test, simply complete steps one through three again.

Note: If you already have an electronic Vaxigo vaccination record, please complete the registration here to consent to share your vaccination status with Konica Minolta .
